PROFESSIONAL EXPERIENCE: Mr. Campise has over 29 years of surveying experience, including 10 years with the Ohio Department of Transportation. His responsibilities with White Shield have involved industrial construction survey, serving as Project Manager for survey services supporting Bechtel National and the US Army in the construction of the Pueblo Chemical Agent Pilot Plant at Pueblo Chemical Depot, CO; and currently as Project Manager for construction survey services supporting Bechtel National and the US Department of Energy for the construction of the Waste Treatment Plant complex at the Hanford Nuclear Reservation. Previously, he served as principal surveyor for HB Engineering on various school projects for the Buckeye Local School District in Rayland, Ohio. Additionally, he supported Bechtel National as Field Survey Engineer for the construction of the William H. Sammis Power Plant at Stratton, OH, for First Energy Corp., and Black & Veatch for construction of the Cardinal Power Plant at Tiltonsville, OH, for American Electric Power. He has demonstrated survey experience in highway construction, ROW, bridges, and was part of a team that worked with the National Geodetic Survey in physically setting and controlling 1st order HARN System monumentation located in Southeastern Ohio. Mr. Campise also worked on the Elkins, West Virginia to Washington, D.C. bypass project as the lead surveyor of verification teams for the State of West Virginia Department of Transportation. Additionally, his background includes setting aerial control, cadastral surveys, and environmental landfill construction surveys supporting the remediation of the Allied Honeywell site in Moundsville, West Virginia. Clients have included the US Army Corps of Engineers Pittsburg District, Departments of Transportation (Ohio, West Virginia, and Pennsylvania), Consolidated Coal, City of Pittsburg, and other state and local agencies and municipalities. Instrument experience includes the operation of several different brands of total stations (Trimble 5600 & S6), digital levels, automatic levels, split bubble centimeter levels for first order (blue book) bench marks, GPS, pin finders, chains, tapes, wheels, SMI software, TDS software, Eagle-point software, MICRO station software, Trimble software, Ashtech software, SMI data collectors, Sokkia SDR33 data collectors, and Wild GRE3 data collectors also Carlson software and Agtech.
RELEVANT EXPERIENCE: White Shield Inc. (2008- to present) Site Administrator for the Waste Treatment Plant in Richland, Washington. Responsibilities include management of up to 28 White Shield surveyors on the project. Provide equipment and staff for construction of nuclear quality project for Bechtel National. As White Shields WTP Site Administrator, Mr. Campise is responsible for the implementation of the Integrated Safety Management System (ISMS) in all work processes, and compliance with Bechtels ES&H procedures, policies, programs, and guidelines. His duties include the following:
Develop personal safety plans for White Shield personnel Monitor the safe work practices of White Shield WTP project personnel Perform safety assessments, accident investigations, causal analysis, compilation of required reports, and conducting training on any safety concerns Conducting hazard evaluations for all phases of work activities, providing mitigative recommendations for hazard control, and ensuring implementation of those controls Implementing programs to ensure compliance with regulatory requirements to reduce injuries accidents, illnesses, fines, and work delays. Attends WTP meetings and participates in the development of safety policy Champions the principles associated with ISMS and the VPP to ensure work is conducted safely, and in accordance with regulatory requirements Remain cognizant of applicable Federal, State, local and customer safety regulations as they apply to the WTP project, and disseminate information on new or revised regulations to White Shield personnel Inspecting White Shield WTP project safety equipment as required by site procedures Support the customers principles of ISMS, VPP, SETO, and STAART Card, and ensure the understanding and participation of White Shield personnel. Perform STAART Card reviews for White Shield personnel to ensure compliance with safe work practices Establish and maintain the White Shield JHA as a living document Maintain and oversee White Shields MSDS Research and purchase all White Shield safety products Lead the White Shield VPP effort, and provide a channel for VPP mentoring programs to facilitate the distribution of learning materials Supply data and support for the annual subcontractor assessment Administrative Functions Enforces discipline policy for White Shield employees at the WTP project. Maintains accurate training records for the LMS schedule for the employees as per the ISMS and 10 CRF 851 project requirements. Attends all required meetings for Bechtel Nation and Department of Energy. Maintains a supervisory role as a liaison for White Shield and Bechtel National. Works as part of the subcontract structure to ensure quality of the documentation and work process structure. Performs quality walk downs as part of the White Shield quality program. Implemented the Certified Survey Technician test and training program corporately. Provides employees with a facility for the off hours study group. Responsible for acquiring testing facility, times and dates of testing, proctor and communicating with employees for proper test taking materials. Tracks and maintains records of all employee certifications and renewals. Provides a point of contact for the WTP security task force. Maintains that Security Protocol is observed by all White Shield Employees. Maintains an extensive, updated database of survey resumes. Reviews all White Shield documentation submitted to Bechtel National. Includes contractual, accounting, daily reports and timesheets. Point of contact for the White Shield Employee Assurance Program. Point of contact for Employee Concerns Program. Maintains equipment logs and required maintenance and repair schedules for White Shield equipment including vehicles dedicated to the WTP project.
White Shield Inc. Subcontract survey lead for Bechtel National at the Pueblo Chemical Depot, Pueblo Colorado (2007-2008). Job duties included supervise crews for construction of project. Calculate layouts for crews, cross check vendor and design drawings for inaccuracies. Provide quality control and quality assurance for project completion. Oversee all subcontract survey work. Provide communication with superintendants and crews for excavations, footings, concrete placement, steel erection, electrical, mechanical placements, utilities and road work. Responsible for setting up control networks for accurate survey data for crews and craft. Provide verification and reports to engineers, superintendants and Bechtel management. Purchase and maintain all Bechtel survey equipment. Provide safety leadership for survey department. Brayman Construction Field Engineer (2007) for a one man GPS crew for layout of Power Plant Piles (Driving & Drilling) in Pittsburgh, PA. Included use of Trimble, S6 & GPS equipment, layout, asbuilts, various other quality assurance and quality control duties. Provide accurate information for micro piles, structure placements, piling locations, utility locates, vertical clearances for crane movement. Document and verify power line locations for OSHA clearance of crane travel. Provide documentation of all underground obstructions; work closely with Bechtel design engineers for critical placement of structures for project completion. Provided locations, elevations and record of pile plumbness for over 6000 ninety foot long piles. Have also recorded test pile results for stressing piles
Senate Engineering As Party Chief, (2006) One man robotic survey crew using Trimble 5600 and S6 units. Construction staking, boundary and topo for a variety of Engineering Projects. Provided locations for auger cast piles for 1000 foot stack erection in power plant. Provided detailed electronic data for Black and Veatch general contractors for easements, utility locations, structure placements, road ways and cell placements for river structures for offloading material to plant. Provided river depth survey data for dredging the Ohio River.
Triangle Surveying and Engineering As Party Chief, (2005) Mr. Campise preformed layout of subdivisions, sewer lines, underground utilities, cul-de-sacs, drives. Grade staking and construction layout of foundations, roads etc. House stake outs, Topo surveys, boundary Topcon with SMI Construction 7 data collector Status-layoff.
Pedersen & Pedersen Party Chief, (2004-2005) Corridor H in West Virginia. Verifying the project status for the State of West Virginia. This section of the project is a $24,000,000.00 2 mile stretch of highway with 3 bridges and curbing along with median. Required to set grade, verify grade, locations and document as built locations. Topcon Instrument with SMI Construction 7 data collection.
Shaw Environmental As a Project Surveyor, (2004) currently operating a robotic survey station(Trimble 5600) to perform various survey functions related to the construction of two landfill cells (2.5 acre and 5 acre) and the excavation and removal of hazardous waste. Survey work includes design layouts, quantity take-offs, and, as built surveys. Work is sometimes performed with Level B, C, and D personal protective equipment. Trimble robotic 5600 unit and ACU controller.
Campise Technical Services As a Self-Employed Surveyor, (2000-2004) Mr. Campise preformed boundary surveys and construction layouts. Researched deeds, plan reading, and design modifications. Mapped and described boundary surveys and subdivisions. Worked with several regional planning commissions in the design phase of subdivision layouts for sewer and water lines. Topcon, SMI data collection, Lietz total Station.
HB Engineering As Survey Crew Leader, (1998-2000) Mr. Campise preformed boundary surveys and construction layouts. Researched deeds, plan reading, and design modifications. Mapped and described boundary surveys and subdivisions. Worked with several regional planning commissions in the design phase of subdivision layouts for sewer and water lines. Also laid out water storage tanks for several municipalities. Managed a 4- to 5-man survey crew. Designated work task for crew, hours work and project completion status. Designed and laid out a 3000-acre subdivision.
Ohio Department of Transportaion Survey Crew Leader, (1988-1998) Mr. Campise preformed work associated with road design and maintenance. Performed property layouts for land acquisition. Performed first order monumentation for United States Geographical Survey. Operated bar code and 10-centimeter split bubble levels for this process. Established a local grid system for GPS control on state plane coordinates. Surveyed for bridge placements and participated in the design process for super elevation for roadway curves and bridges.
Ziemba, McDivitt and McCoullagh, Mr. Campise, (1981-1988) preformed tasks associated with front and rear chainman (rodman). Corner locations i.e. using a pin finder to locate survey monuments. Cut brush and pulled chain for measurements. Performed calculations and drafting of plan layouts.
